What you should do before and at your appointment
prior to first appointment
Please have your medical records, including records from your specialists, sent to Your Choice Medical, llc prior to your first appointment.
Bring your insurance card and photo ID to your first appointment. We are contracted with most major insurance plans, but please check with our office to verify we accept your insurance. MassHealth is accepted and the membership fee will be waived in accordance with 130-Mass.Reg.450.201 for patients with this insurance plan only.
at your first appointment
Call your insurance before your first appointment and switch your primary care to Kay A. Ficht, MD at his new office, Your Choice Medical, llc
Have a method of payment for the membership fee if you have not done so already
Membership Fee
-Payment schedules are annually or quarterly (April 1st, July 1st, October 1st, Jan 1st)
-Credit
card, check, or cash will be accepted.
-We reserve the right to terminate any membership due to the non-payment of the membership fee
